Companionship

Support and connection for your loved one. Time together could include conversation, walks, community outings, or everyday activities, all tailored to their interests and comfort-level. The goal is to support emotional well-being, reduce isolation, and enhance overall quality of life.

Care Planning

Consultation to develop a care plan for next steps. Support could include facilitating family discussions, navigating the healthcare system, understanding available resources, and exploring living arrangements. The goal is to prioritize an individual’s dignity, autonomy, and goals.

About Tucker

I received my Master’s in Social Work from the University of North Carolina in 2023. Specializing in working with older adults, I have social work experience working in Continuing Care Retirement Communities (CCRCs) and hospice agencies.

I also have experience as a companion, providing company and supervision during medical visits, emotional support and compassionate reflection in one-on-one appointments, and social interaction and cognitive stimulation to individuals living with dementia. I have worked to coordinate complex care needs across interdisciplinary teams to ensure residents’ and patients’ goals are being met. Additionally, I have worked with families to develop care plans for their loved ones, balancing the views and concerns of all members of the family unit.

A gerontologist that I worked with at hospice told me that every phase of life has a primary skill or focus. For our older adult years, that skill is adaptation. There are so many changes we encounter as we age, and it can be beneficial to have the company and experience of one who has walked along others as they have made these transitions.

I also have personal experience planning and coordinating care for my own two aging parents. Their care needs are growing and becoming increasingly complex. All the while also balancing raising my own triplet pre-schoolers. It is a journey that requires flexibility, humility, and patience. In my time doing this work with my family and with others, I have lived the difficulty of these challenges. I have also felt the love, support, and closeness that grows from working together through this new phase of life.
